IPOST: Ensuring Patient Voices Are Heard and Wishes Honored

End-of-life is one of the greatest times of transition in life. Advance planning and coordination of care are pivotal in ensuring that appropriate care is received and best outcomes achieved. The Iowa Physicians Orders for Scope of Treatment (IPOST) offers an opportunity to support patients, families and healthcare providers in discussing, defining and documenting patient care goals and preferences. This presentation will feature palliative care leaders and IPOST champions who will offer insight into a comprehensive approach to end-of-life care and the role of IPOST in facilitating the process.

  • Discuss the importance and role of end-of-life care as part of person-centered, value-based care, including unique considerations for care transitions and avoidance of unnecessary, unwanted readmissions.
  • Summarize the utilization of IPOST, including initiation, implementation and execution.
  • Describe the community approach to end-of-life care planning and execution of IPOST to ensure that appropriate and desired care is received in the appropriate and desired setting.

This course, in its entirety, will take an estimated 55 minutes to complete.

Jane Dohrmann
Jane Dohrmann, MSW, LISW, ACHP-SW, is the Program Director of Palliative Care Services which includes the Supportive Care and Honoring Your Wishes programs at Iowa City Hospice. She has worked at Iowa City Hospice since 2003. Dohrmann was recognized as a ChangeAGEnt through the John A. Hartford Foundation for her collaborative work with IPOST implementation in local nursing homes. She is a state representative to the national POLST organization and serves on the NHPCO Palliative Care Council.

 

Tom Mousser
Tom Mouser, MD, is the Chief Medical Officer of EveryStep, the care and support services organization formerly known as HCI Care Services & Visiting Nurse Services of Iowa. He is dual boarded in Internal Medicine and Hospice and Palliative Medicine after completing residency at University of Iowa-Des Moines and Fellowship training at Stanford University. As a palliative care physician, Dr. Mouser has much experience in facilitating discussions with patients and families regarding their understanding of their illness and planning related to their goals of care. He greatly values the IPOST form and sits on the Iowa IPOST Task Force to help inform and facilitate its use.


Kady Reese
Kady Reese, MPH, CPHQ, is the Director of Education & Engagement with the Iowa Medical Society, leading curriculum design focusing on evidence-based skills application and helms strategic operations aimed at development of cross-collaborative initiatives to advance the practice of medicine and patient outcomes. Her experience spans across the healthcare continuum, including home health, primary and urgent care practice, healthcare administration and quality reporting. Reese holds a master’s degree in public health from Benedictine University, as well as certificates in health management and policy and health education and promotion. She served as a national champion for Person and Family Engagement with CMS as part of the Partnership for Patients and the Transforming Clinical Practice Initiatives.

No continuing education is provided for this course. The user will receive a certificate of completion upon completing the course.
