Social Determinants of Health: Impact on Population Health

This on-demand webinar is intended as a brief overview discussing the impact of social determinants of health (SDOH) on population health outcomes. The COVID-19 pandemic has highlighted gaps in health equity and increased individual and community SDOH needs. Loneliness, social isolation, food insecurity, homelessness, and unemployment are a few examples. Clinical and community partners working together across multiple sectors is required to address SDOH needs. This webinar will highlight numerous best practice models and resources available to support integrated efforts. It will also describe the importance of standardizing documentation utilizing ICD-10-CM Z codes. Numerous supplemental resources are also shared as a reference. The resources include a comprehensive SDOH toolkit, SDOH Z code factsheet and for quick access, a summary document listing related iCompass posts.

Cari Seddon, MA, RDN, LD, CDCES

Cari Seddon is the director of community quality initiatives at the Iowa Healthcare Collaborative. In this role she supports collaborative clinical and community partnerships driving sustainable healthcare quality improvements with focused attention on providing person-centered care throughout Iowa. Seddon received a bachelor’s degree in dietetics from the University of Wisconsin-Stevens Point and a master’s degree in family and consumer science from Western Michigan University. Seddon is a registered dietitian and certified diabetes care and education specialist. She is six sigma lean certified and has more than 19 years of experience in patient care and population health management.
