Falls are a serious patient safety issue affecting Iowans everyday. In 2016 in Iowa alone, there were nearly 88,000 falls-related emergency department visits that resulted in 8,300 hospitalizations. Between 2007 and 2016, there was a 56 percent increase in the number of falls-related deaths. We know that most falls are preventable. To protect patient safety and patient lives, we must work together to prevent falls in all care settings. Participating in this pre-recorded webinar course holds many benefits.

As the most common preventable injury, Falls in the acute care setting are a significant concern for hospitals nationwide. These incidents can lead to injuries, longer hospital stays, and increased healthcare costs. In 2024, Cass Health implemented a patient fall debrief program. Whenever a patient falls, a debrief is held with key stakeholders to discuss what happened and look for process improvement opportunities.

The Readmissions Team was restructured at that time, adding team members and returning to basic best practice interventions. Social drivers of health (SDOH) are underlying social and economic conditions that influence an individual's ability to be healthy. Examples include lack of stable housing, limited access to healthy food, loneliness, and feeling unsafe at home. Understanding an individual's SDOH allows for the development of a comprehensive treatment plan leading to improved health outcomes.
